What to bring when ready to check-In

  • All your players and one team representative
  • E4 generated roster or a Fox and the Hare Tournament roster signed by Regional Commissioner
  • Four prefilled lineup cards
    • Each card must include jersey # and players’ full name
    • Each card should be in jersey # order
    • All players listed on the lineup card must be listed on the signed roster and haveAYSO photo ID or AYSO E4 generated photo roster.
    • Any player on lineup card that is not listed on the signed roster will be marked ineligible to play

  • Signed guest player form (only if guest players are joining your team.
  • Signed “Parent/Guardian Consent and Player Medical Release Form”

Check-In Procedure

Be prepared to check-in 45 minutes prior to your first scheduled match.

During check-in all four lineup cards will be stamped. Prior to each game, please hand one of your stamped lineup cards to the match officials.
Only stamped lineup cards will be accepted. If you lose one, return to the check-in tent with your roster and we can provide a blank one to be filled out.

Each player will receive a hand stamp at check-in. If any player’s hand stamp is not visible at any point during the tournament, return to the check-in tent with proper ID to receive a new one. Match officials will be checking for hand stamps prior to each match.

All teams must have photo ID/photo roster (for players and coaches) and medical release forms with them at all times and be prepared to present them to tournament or match officials upon request.
